Service Coordination
Service Coordination is the foundation for all services delivered or coordinated by Horry County Disabilities and Special Needs.
The determination of eligibility for services is the responsibility of the South Carolina Department of Disabilities and Special Needs in Columbia, but HCDSN Service Coordinators assist consumers through the eligibility process. Once an individual is eligible for services, the Service Coordinator plays a key role in assisting the consumer with needs identification and with locating the appropriate resources for filling those needs.
Consumer choice and the concept of the "least restrictive environment" is central to this process.
Making the eligibility process go more easily
In preparation for your initial home visit to begin services, it would be helpful if you had the following information on hand. (Note: Not all items will be applicable for everyone). If you do not have access to a copier, we can copy these documents in our office and return the originals to you.
- All psychological evaluations
- School records (IEP)
- Other agency reports (Mental Health, previous programs attended)
- Medical reports
- Therapy reports (Speech, OT, PT)
- Copy of Birth Certificate
- Copy of Social Security Card
- Copy of Medicaid and/or Medicare cards
- Copy of Private Insurance card
- Names, complete addresses and telephone number of:
- Doctors
- Dentists
- Schools attended
- Other Special Needs programs.
